- All reports, biopsy results and scan CDs
- Old prescriptions and discharge summaries
- ID and Ayushman card if you have one
Notes
- A list of medicines with doses
- Your questions, written down
- Phone numbers of your doctors
Tip
- Keep copies, and photos on your phone
- Arrange them by date in one folder
